Southern Jersey Family Medical Centers, Inc. is looking for candidates with talent, professionalism and compassion. We are currently seeking Community Health Workers to improve health outcomes in Atlantic County, Burlington County and Salem County.

Summary: The Community Health Worker will work as a part of the Community and Patient Engagement Team (CAPE) and will go out into the community to educate and work with specific target populations. Ideal candidate should have prior experience or education in health and/or social services. Local travel is required.

Hours: This is a full-time position that requires the ability to work a flexible schedule.

Position Responsibilities/Duties:

• Community outreach to promote the programs and  services of SJFMC

• Covid-19 outreach; includes assisting with site operations, patient education, and testing
• Establish positive, supportive relationships with community partners
• Working with the community to increase engagement in initiatives, as well as patient recruitment and retention within the health centers
• Effectively work with our diverse patient populations to address social determinants and identify needs through screenings and assessments
• Organize and coordinate outreach events
• Encourage patients to be active, engaged participants in their health
• Public speaking and presentation skills to diverse groups of individuals, including community partners, meetings and events
• Continuously expand knowledge and understanding of community resources, services and programs provided

Other duties as assigned

Experience and Skills:

• High school diploma or equivalent
• Health/social services experience or education
• Strong verbal and written communication skills
• Creative approach to problem-solving
• Computer skills: EPIC experience preferred
• Valid driver’s license and reliable transportation
• Bilingual preferred
